Expect me to be in your home approximately 1.5 to 2 hours. When I arrive, we will briefly review your previously completed health history and intake form. If your baby is ready to eat when I arrive, this can wait until a better time during my visit. Your baby is most important. We will do a weight check. I will assess you & your baby feeding. We will then discuss and formulate a plan of care for feeding your baby. If necessary, we may schedule a follow-up appointment at this time. I will send a report summary of our visit to your primary care provider(s).

Currently, I am not in network with any insurance companies. However, after our consult, I will provide you with a SUPERBILL for you to submit to your insurance company for reimbursement. This is an itemized receipt of my services. The superbill will contain all of the information necessary information needed for reimbursement. This is a benefit of the Affordable Care Act (ACA). The National Women’s Law Center has a toolkit available to you for understanding your rights to breastfeeding/lactation services under the ACA.

It is up to you to know & understand your insurance plan and policies. If you are looking to be reimbursed for my services, before booking an appointment, I suggest calling your insurance company and inquire about coverage for out-of-network Lactation support. 

Virtual Appointments: If you are looking to be reimbursed by your insurance provider for our appointment, please check with your insurance provider to inquire about your specific plans benefits before booking. Some insurance providers are not covering or reimbursing for virtual lactation consultations. 

A credit card is required at time of booking via all major credit cards (VISA, Mastercard, Discover, AMEX) or debit cards to reserve your appointment. This includes using your FSA or HSA card! Your card will be charged the listed service fee at the time of our visit together. 

Yes! Your FSA or HSA card is accepted through IntakeQ Pay when you book your appointment.

Thank you for considering my services! It really is up to your comfort level. While we can achieve a lot virtually, a hands-on, in person appointment will allow me to do a thorough assessment of you & your baby. Please e-mail hello@heathermarielactation.com for specific inquires. 

Yes! We will use a HIPPA compliant secure platform to conduct our consultation. 

Refunds will not be offered for any undesired breastfeeding outcomes. 

In the event that I need to cancel your appointment for situations beyond my control, you will receive a full refund. I will make every attempt to re-schedule your appointment. 

Should you cancel less than 24 hours prior to your scheduled appointment time, you will be refunded all but a $50 late cancellation fee. 

Before canceling, contact me if you need to change your appointment date/time or simply wish to reschedule. 

For a full refund, please make every attempt to cancel your appointment as soon as possible, prior to 24 hrs before the appointment. This is to accommodate other families waiting for an appointment. 

If you cancel less than 24 hrs prior to your appointment time, you will receive a refund for the appointment amount less a $50 late cancellation fee. 

If you have questions, concerns or challenges related to feeding your baby, I would love to help you. Please consider an appointment. Unfortunately, I am not able to answer specific questions or offer advice related to your situation without proper consent and knowledge of your health history.

After you book your appointment, you will receive a consent form & intake form that is required to be submitted & completed prior to your appointment. This will allow me to prepare beforehand and give us more time to focus on your feeding concerns. Please do your best to complete this as soon as possible. 

If your consent & intake form are not completed 48 hours prior to your appointment, I will send you a reminder. If the forms are not completed 24 hrs to our appointment, your appointment will be cancelled. 

I will reach out to you if I have specific questions or requests prior to our appointment. 

After you book your appointment, you will receive a consent form & intake form that is required to be submitted & completed prior to your appointment. This will allow me to prepare beforehand and give us more time to focus on your feeding concerns. 

You are going through a lot right now. If you have specific questions or concerns for me, please write them down and we can address them during our visit. 

You will receive an appointment reminder three days before and the morning of our appointment. You will also receive detailed information on how to prepare for my visit. 

Have your breast pump accessible in the event that we need to use it. 

Your baby will need to be present for our consultation if we are working on feeding challenges. 

I love animals. However, your pets love you & may become protective over you. Please have your pets secured safely in another room for my safety & to avoid disruption during our consult. 

Your feeding concerns are important to me. Please ensure privacy during our appointment. Please refrain from visitors during this time. 

PLEASE for everyone’s safety: if you are someone in your household has been sick (symptoms or fever within the last 24 hours) consider re-scheduling your appointment or canceling until everyone is healthy.  

 

Yes! I am up-to-date on all of my vaccinations.

I am a Registered Nurse in the State of Michigan with a Bachelors of Science in Nursing from Grand Valley State University. I am a International Board Certified Lactation Consultant. 

I am current with my CPR & NRP certifications. 

I am fully vaccinated against COVID-19. My supplies I use are sanitized & cleaned with virucidal disinfectants before & after every appointment. PLEASE, if you, or a family member in your household have been feeling ill, have had a fever or any symptoms, or recently exposed to COVID-19, contact me for re-scheduling our appointment. This is for the protection of yourselves, myself and all of the families and little ones I serve. Thank You for your understanding!
